Reasonable Adjustment Flag

A Reasonable Adjustment Flag is a note on your health and care records that tells NHS and care staff that you may need some changes or extra support to access healthcare safely and fairly. The criteria for this is defined in the Equality Act 2010 as “A physical or mental impairment that has a substantial and long-term (greater than 12 months) negative effect on ability to do normal daily activities”. You may need help if you have a disability (body, sense or learning), Autism or A long -term condition (like dementia). Please let us know if you require a reasonable adjustment flag adding to your medical record. Examples of reasonable adjustments include:

  • Allowing extra time for appointments.
  • Providing information in easy-read format.
  • Allowing extra time for appointments.
  • Offering a quiet waiting area.
  • Using a British Sign Language (BSL) interpreter.
  • Communicating through a carer, supporter, or advocate when appropriate.
  • Making sure buildings and equipment are accessible.
