From Thursday 26th May the surgery will be closed every Thursday lunchtime for 1 hour, from 1pm to 2pm, to allow time for staff training.  We are open as usual on all other days.  If you have a medical issue that cannot wait while we are closed, please call the usual surgery number, 01726 822254, and your call will be forwarded to the urgent doctor service.

New Patient Registration

Please allow 5 working days from when you submit your registration forms to booking an appointment.register_now

Our practice area covers the villages of St Stephen, St Dennis, Roche, Foxhole, Nanpean, Whitemoor, Treviscoe, Indian Queens, Fraddon, Summercourt, Ladock, Probus, Grampound Road, Grampound, Hewas Water, Sticker, Polgooth, Coombe, Lanjeth and the surrounding areas.

New Patient Questionnaire

Registration forms and a new patient questionnaire are available from the receptionists or can be downloaded from the above documents.  You will also need to provide us with ID and a utility bill as proof of identity (we will be unable to register you until we have seen these documents)

Patients may express a preference as to which doctor they register with however, we do not operate personal lists so you may be offered an appointment with any doctor
