New Patient Registration

Please allow 5 working days from when you submit your registration forms to booking an appointment.

Our practice area covers the villages of St Stephen, St Dennis, Roche, Foxhole, Nanpean, Whitemoor, Treviscoe, Indian Queens, Fraddon, Summercourt, Ladock, Probus, Grampound Road, Grampound, Hewas Water, Sticker, Polgooth, Coombe, Lanjeth and the surrounding areas.

GMS1 Registration Form

Registration forms are available from the receptionists or can be downloaded from the above document.  You will also need to provide us with ID and a utility bill as proof of identity (we will be unable to register you until we have seen these documents)

Patients may express a preference as to which doctor they register with however, we do not operate personal lists so you may be offered an appointment with any doctor
